In this article, we propose a value-based care operating model within the context of organizational maturity. Provider engagement with value-based care is somewhat limited. Stage of Maturity Requires ConsiderationOptimizing performance in a value-based care environment requires management to focus on continuous improvement in their clinical, operational and financial organizations. A clinical focus on value-based care requires member and provider engagement, fewer gaps in care and proactive intervention to reduce ED visits, hospitalizations and re-hospitalizations.
