Poor record keeping and communication problems meant a newborn baby boy had to face surgery, after his symptoms were initially dismissed. A report from the Health and Disability Commissioner, released this week, found failures in record keeping and communication by the MidCentral District Health Board resulted in the inadequate care. At the time, MidCentral was testing a new electronic health records system, which meant staff were electronically recording clinical decisions, but not necessarily the thinking behind diagnoses. He recommended the DHB report back to the Health and Disability Commissioner on the implementation of recommendations arising from its own "root cause analysis" report.
