Stephen Carré, of Eaton Bray, Bedfordshire, died after the Department for Work and Pensions rejected his appeal against the decision. The DWP carried out 144 internal reviews between 2012 and 2019, with all cases involving people claiming support who died or came to serious harm. Copies of internal reviews launched after July 2019, and seen by the BBC Shared Data Unit, suggest fatal mistakes were continually being made.
