Loss of patient records at Queen Mary Hospital

Names, HKID numbers, addresses, telephone numbers and service records of 442 patients who received "Integrated Care and Discharge Support" services in year 2012 and 2013 were lost by Queen Mary Hospital. Those patients involved will be notified by the hospital. The hospital said a clerical staff reported that one steel filing cabinet could not be located in the storeroom last Friday. The cabinet has been locked and was used to store service records pending destruction, including the information of 442 patients. The hospital has notified the Hospital Authority Head Office through the Advance Incident Reporting System and the Office of the Privacy Commissioner for Personal Data.
