Phillip Hughes. The death of cricketer Phillip Hughes was a "tragic accident" and no-one was to blame for the tragic outcome, a coroner has found. Members of the Hughes family, who were emotional during the five-day inquest, were not in the courtroom as the findings were handed down. Mr Barnes said it was obvious the Hughes family did not believe all the evidence heard during the inquest. Cricket Australia high performance chief Pat Howard said the organisation's thoughts continued to be with the Hughes family.
