|The heartbreaking final moments of a father who died during a tragic 'crush death' after getting stuck in an indoor cave at a birthday part have been revealed. Carl O'Keeffe (pictured), 49, died at an attraction in the Lake District where other tourists had previously 'got stuck.' Cumberland Council has since confirmed it would be investigating how the tragedy happened.
