Fatality inquiry begins into mental health caregiver's death

Officials from the Canadian Mental Health Association and Persons with Developmental Disabilities are expected to testify at the fatality inquiry. Inquiry lawyer Kirk Lambrecht said he plans to examine what information was shared between mental health agencies before Wolski was killed on Feb. 12, 2011. Another worker went to the Camrose duplex the next morning to relieve the mental health caregiver, and found her dead on the floor in the living room. She said WDACS wasn't told when Saddleback was eventually released from hospital and the Canadian Mental Health Association assumed responsibility for him. A preliminary provincial investigation has already established the Red Deer office of PDD failed to pass along information about Saddleback's history and risk to the mental health association.
